Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who works independently to manage patient care in PALTC settings, aiming to improve outcomes, enhance the healthcare experience, reduce hospitalizations, and lower costs.
Responsibilities include conducting physical exams, diagnosing and treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and external providers. The AP works autonomously, making clinical decisions within scope, while building strong relationships with leadership and interdisciplinary teams.
They ensure compliance with regulations, maintain accurate documentation, participate in quality improvement, and promote health and safety protocols. The role requires excellent communication, decision-making, and proficiency with electronic health records and telemedicine tools. Candidates should have a relevant degree, at least 2 years of experience, board certification, and an active license.
