Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time, in-person role for an APRN or Physician Assistant with at least 2 years of experience, preferably in long-term care.
They work independently, collaborating with facility leadership and teams to manage patients’ comprehensive medical needs in PALTC settings.
Responsibilities include conducting examinations, diagnosing and treating illnesses, developing care plans, prescribing medications, and providing preventive care.
They build strong relationships with staff, families, and external providers, ensuring compliance with regulations and maintaining accurate documentation.
Key skills include clinical decision-making, effective communication, technology proficiency (EHR, telemedicine), and the ability to work autonomously in urgent situations.
