Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person full-time role for an APRN or PA working independently in PALTC facilities.

Responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, and providing preventive care.

They collaborate with facility leadership and interdisciplinary teams, ensuring compliance with regulations, maintaining documentation, and participating in quality improvement.

The AP manages patient care independently, making clinical decisions, and acting as the primary provider within the facility.

Strong communication, clinical decision-making, and technology skills are essential, along with physical ability to respond quickly in emergencies.

Qualifications include a relevant degree, at least 2 years of experience, board certification, and long-term care experience preferred.

Compensation ranges from $66,948 to $120,000 annually, with benefits such as health insurance, 401(k), paid time off, and more.
