Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person role for an APRN or PA, working independently to manage patient care in PALTC settings.
Responsibilities include conducting exams, diagnosing, treating, prescribing medications, and developing care plans to improve outcomes, reduce hospitalizations, and enhance patient and family experience. Collaborates with facility staff and external providers, ensuring compliance with regulations, maintaining accurate documentation, and participating in quality initiatives. Provides preventive care, manages chronic conditions, and promotes health and safety protocols. Requires strong clinical decision-making, communication, and technology skills, with the ability to work independently in a fast-paced environment.
Qualifications include a relevant degree, 2+ years of experience (long-term care preferred), and board certification.
