Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for delivering independent, comprehensive medical care in PALTC facilities.
Provide physical exams, diagnose, treat, and manage acute and chronic conditions, and prescribe medications. Develop care plans, conduct preventive services, and monitor patient health. Collaborate with facility leadership and interdisciplinary teams to ensure holistic patient care. Maintain compliance with regulations, document accurately, and participate in quality initiatives. Promote infection control, health education, and safety protocols. Work independently with strong clinical decision-making skills, effective communication, and proficiency with EHR and telemedicine tools. Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure.
