Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for delivering independent, comprehensive medical care in PALTC facilities.

  • Provide physical exams, diagnose, treat, and manage acute and chronic conditions, and prescribe medications.
  • Develop care plans, conduct preventive services, and monitor patient health.
  • Collaborate with facility leadership and interdisciplinary teams to ensure holistic patient care.
  • Maintain compliance with regulations, document accurately, and participate in quality initiatives.
  • Promote infection control, health education, and safety protocols.
  • Work independently with strong clinical decision-making skills, effective communication, and proficiency with EHR and telemedicine tools.
  • Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure.
