Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), with specialized wound management training.

  • They assess, treat, and develop individualized wound care plans for residents, including advanced procedures like debridement and negative pressure therapy.
  • Collaborate with interdisciplinary teams to prevent wounds, reduce complications, and improve outcomes while ensuring regulatory compliance.
  • Provide staff education, resident and family engagement, and participate in quality improvement initiatives.
  • Maintain detailed documentation, manage infections, and serve as a clinical resource.

    Qualifications include a relevant degree, 2+ years of experience, board certification, and preferred specialty certification in wound care.
