Job Description
The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.
Key duties include conducting comprehensive assessments, developing individualized care plans, performing advanced wound treatments, and collaborating with healthcare teams to optimize outcomes.
The role emphasizes wound prevention, staff education, documentation, quality improvement, infection control, and regulatory compliance, while advocating for resident dignity and engaging families.
Qualifications require a relevant degree, at least 2 years of experience, board certification, and preferred specialty wound care certification. Strong communication, leadership, and proficiency with electronic health records are essential.
