Job Description
The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), with specialized wound management training.
They are responsible for
Assessing and managing wounds in long-term care residents, including prevention and treatment of various wound types. Developing personalized care plans and performing advanced wound therapies. Collaborating with interdisciplinary teams to ensure comprehensive care. Providing staff education, promoting prevention strategies, and ensuring regulatory compliance. Monitoring outcomes, participating in quality improvement, and consulting with residents and families. Addressing infections and advocating for resident needs, with strong communication and technological skills. Qualifications include a relevant degree, 2+ years of experience, board certification, and preferably specialty wound care certification.
