The Nursing Home Wound Care Advanced Practitioner (AP) is a licensed Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.

They aim to reduce complications, improve care quality, enhance resident and family experience, lower preventable hospitalizations, and decrease costs.

Key duties include comprehensive assessments, developing individualized care plans, performing advanced wound treatments, collaborating with healthcare teams, implementing prevention protocols, providing staff education, maintaining detailed documentation, participating in quality improvement activities, and ensuring regulatory compliance.

They serve as a clinical resource, advocate for residents, and educate families.

Strong communication, leadership, technology proficiency, and physical ability are essential.

Qualifications include a relevant degree, at least 2 years of experience, board certification, and preferred specialty certification in wound care. This full-time role offers comprehensive benefits and requires in-person work.
