Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional—either a Nurse Practitioner (NP) or Physician Assistant (PA)—specialized in wound management within a PALTC facility.

Their primary responsibilities include comprehensive resident assessments, developing and revising individualized wound care plans, performing advanced wound treatments, and collaborating with interdisciplinary teams to ensure optimal care.

They focus on wound prevention, staff education, documentation, quality improvement, infection control, and regulatory compliance.

Additionally, they serve as a clinical resource, advocate for residents, and engage families in wound care education.

Qualifications include a relevant degree, at least 2 years of experience, board certification, and preferred specialty certification in wound care.
