Job Description

The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care services under physician supervision, focusing on wellness exams, acute illness visits, and disease management.


  • Assess, diagnose, and manage patients with both acute and chronic conditions, emphasizing prevention and health education.
  • Collaborate with a healthcare team to coordinate holistic care, especially for underserved populations facing social barriers.
  • Maintain accurate documentation in the EHR and participate in quality improvement and care initiatives.
  • Support health promotion and disease prevention efforts tailored to community needs.
