Job Description

The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care under physician supervision, focusing on wellness exams, acute illness, and disease management.

Key Responsibilities:

• Deliver comprehensive care across all ages, including preventive services and chronic condition management.
• Collaborate with a multidisciplinary team to ensure holistic patient care.
• Maintain accurate documentation in the EHR and participate in quality improvement initiatives.
• Support health promotion and disease prevention, especially in underserved communities.

Qualifications:

• Certified Family Nurse Practitioner (required).
• Preferred 1 year of experience in HIV care, primary care, or community health.
