Certified Family Nurse Practitioner (1 yr exp preferred)

Job Description

The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care under physician supervision.

Assess, diagnose, and manage patients across all ages, focusing on wellness, acute illness, and chronic disease management.

Collaborate with a multidisciplinary team to ensure holistic care and coordinate services.

Maintain accurate documentation in the electronic health record and participate in quality improvement initiatives.

Support health promotion and disease prevention, especially in underserved communities facing social barriers.

Requirements: Certified Family Nurse Practitioner license and 1 year of experience preferred in primary, HIV, or community health.
