Job Description
The Certified Nurse Practitioner in the Nursing Home serves as a key member of the Interdisciplinary Care Team, providing care management, clinical assessments, and education to members, families, and staff.
Their primary responsibilities include triaging care needs using HRAT scores, conducting ongoing clinical assessments, facilitating access to various healthcare services, evaluating patients post-hospital discharge, and initiating appropriate treatments. They also advocate for beneficiaries by educating on self-management and support communication among care team members.
This full-time role involves visiting 1-2 nursing homes daily, with a Monday-Friday, 8am-4:30pm schedule, promoting a healthy work/life balance.
Requirements include a valid Ohio Nurse Practitioner license, at least 3 years of healthcare experience (preferably in HMO/PPO settings), and no license suspensions or exclusions from Medicaid/Medicare.
Compensation ranges from $110,000 to $115,000 annually, with benefits such as 401(k), insurance options, paid time off, tuition reimbursement, and flexible scheduling.
