Job Description

The Certified Nurse Practitioner in the Nursing Home serves as the key member of the Institutional Interdisciplinary Care Team, providing care management, clinical assessments, and education to residents, families, and staff.

Conducts ongoing clinical evaluations, triages care needs based on HRAT scores, and facilitates access to various healthcare services.

Visits residents in 1-2 nursing homes daily, especially after hospital discharges, to assess progress, perform exams, prescribe interventions, and communicate findings.

Educates beneficiaries and families on condition management and advocates for their needs.

Works Monday-Friday, 8am-4:30pm, ensuring a healthy work/life balance.

Requirements: Valid Ohio NP license, 3+ years healthcare experience, preferably in HMO/PPO settings.

Compensation & Benefits: $110K-$115K annually, with comprehensive benefits including insurance, 401(k), paid time off, tuition reimbursement, and flexible scheduling.

Position involves travel and on-site visits to nursing facilities.
