Job Description
The Certified Nurse Practitioner in a Nursing Home serves as a key member of the Interdisciplinary Care Team, providing care management, clinical assessments, and education to members, families, and staff.
Responsibilities include triaging care needs using HRAT scores, conducting ongoing assessments, facilitating access to various services, evaluating patients post-discharge, and managing episodic complaints with appropriate interventions.
The role emphasizes patient advocacy, education on condition management, and collaboration with other practitioners. It requires a valid Ohio NP license, 3+ years of healthcare experience, preferably in HMO/PPO settings.
This full-time position offers a work schedule Monday-Friday, 8am-4:30pm, with a salary ranging from $110,000 to $115,000 annually, and benefits such as health insurance, 401(k), paid time off, and tuition reimbursement.
