Job Description

The Provider Lead is a practice-based clinical role supporting members through scheduled, walk-in, and urgent visits across various settings (telephonic, virtual, clinic).

They deliver comprehensive care including chronic disease management, behavioral health conditions, and preventive wellness, collaborating with an interdisciplinary team to develop and implement care plans.

The position oversees complex patient care, serves as the escalation point, and ensures high-quality, safe, and effective treatment by direct reports.

This hybrid role requires an MD/DO with Board Certification in Family or Internal Medicine, 5+ years of member care experience, and 2+ years of informal clinical leadership.
