Job Description

We are seeking a qualified Registered Nurse (RN) to provide comprehensive patient care in a home health setting.

  • Conduct patient assessments, monitor vital signs, and evaluate health histories.
  • Administer medications and treatments safely and as prescribed.
  • Deliver specialized care for internal medicine, dementia, and dialysis patients.
  • Perform nursing procedures such as catheterization and airway management.
  • Collaborate with healthcare teams to develop personalized care plans and complete MDS documentation.
  • Educate patients and families on health coaching strategies to improve outcomes.
  • Support surgical teams in the operating room, ensuring sterile conditions.
  • Maintain accurate, compliant patient records.

    • Qualifications include a current RN license, home health experience, and preferred background in dementia care, internal medicine, or dialysis. Strong communication skills and the ability to work in a team are essential. The role offers full-time or part-time hours with a flexible schedule, paying $40-$50/hour. Candidates should be able to work in Fort Wayne, IN, and have at least one year of dementia care and home health experience preferred.
