Seeking a full-time Home Care RN Clinical Manager to lead and supervise a home health care team, ensuring quality patient care aligned with agency standards and regulations.

• Oversee clinical activities, monitor team performance, and ensure compliance with Medicare and state regulations.
• Manage patient cases, review documentation, and approve care plans and discharge summaries.
• Facilitate quality improvement initiatives and coordinate patient schedules for optimal care delivery.
• Provide clinical oversight and support team development within a compassionate, patient-centered environment.

Requirements include at least 2 years of experience in a licensed Medicare home health agency, a valid Colorado RN license, and basic computer skills.
