Job Description

We are seeking a full-time Home Care RN Clinical Manager to lead and supervise a home health care team, ensuring high-quality patient care and compliance with regulations.

  • Provide clinical oversight, monitor team performance, and ensure competency in delivering home health services.
  • Manage case evaluations, treatment plans, and documentation to meet quality standards and optimize patient outcomes.
  • Review and approve electronic documentation, including OASIS and Plan of Care, ensuring accuracy and compliance.
  • Participate in quality improvement activities and coordinate patient care schedules.
  • Lead team development, implement care models, and ensure adherence to agency policies.


    • Requirements include at least 2 years of home health RN experience, knowledge of Medicare and state regulations, basic computer skills, and a valid Colorado RN license. Benefits include 401(k), health/dental/vision insurance, paid time off, and life insurance. The role offers a salary of $90,000-$94,000 annually.
