Job Description
The Advanced Practice Provider (APP) provides comprehensive outpatient primary care across all ages under supervision in a Medical Home model.
Responsibilities include evaluation, diagnosis, treatment, coordination of services, and documentation of patient encounters. Creates integrated treatment plans involving behavioral health, pharmacy, therapy, nutrition, and surgery. Participates in a shared call rotation, including evenings, weekends, and holidays. Collaborates with multidisciplinary teams to ensure quality, evidence-based care and optimal outcomes. Engages in patient education, supports quality improvement initiatives, and maintains compliance with licensing, certifications, and hospital policies. The role requires a Master’s degree, valid licensure, relevant experience in outpatient Family Medicine, and strong communication skills. Preferred candidates have 2+ years of experience, familiarity with Medical Home models, and Epic EHR experience.
