Job Description
Seeking an Advanced Practice Provider to deliver comprehensive outpatient primary care at a Family Care Clinic in Prineville, OR.
They will evaluate, diagnose, treat, and document patient encounters across all ages, coordinating services within a Medical Home model to optimize outcomes and cost-effectiveness.
Work involves creating integrated treatment plans, monitoring ongoing care, and referring patients to specialists as needed.
Schedule: Minimum 40 hours/week with patient contact of at least 36 hours; participation in a shared call rotation including evenings, weekends, and holidays.
Responsibilities include practicing evidence-based family medicine, collaborating with multidisciplinary teams, prescribing medications, providing patient education, and ensuring accurate documentation.
Qualifications: Master’s in Physician Assistant Studies or Nursing, valid licensure, certifications, and outpatient experience; preferred experience in Family Medicine and familiarity with Medical Home models.
