Job Description

The Advanced Practice Provider (APP) provides comprehensive outpatient primary care under supervision in a Medical Home model at the Prineville Clinic.

They evaluate, diagnose, treat, and document patient encounters across all ages, coordinating services like behavioral health, pharmacy, and specialty referrals.

The role includes practicing evidence-based family medicine, prescribing medications, and supporting preventive and follow-up care.

APPs participate in call rotations, support quality improvement initiatives, and collaborate with multidisciplinary teams to ensure optimal patient outcomes.

Qualifications include a Master’s in Physician Assistant Studies or Nursing, current licensure, certification, experience in outpatient family medicine, and proficiency with EHR systems.

Full-time position with benefits, requiring relocation to Prineville, OR, and a minimum of 40 hours/week with scheduled patient contact and call duties.
