Job Description
The Advanced Practice Provider (APP) provides outpatient primary care in a Family Medicine Medical Home model under supervision, delivering comprehensive evaluation, diagnosis, treatment, and documentation for patients of all ages.
They coordinate with multidisciplinary teams, including behavioral health, pharmacy, and care management, creating personalized treatment plans and monitoring patient progress. The role involves prescribing medications, interpreting diagnostics, patient education, and participating in after-hours call rotations.
Located at the Prineville Clinic, the APP works a minimum of 40 hours/week with scheduled patient contact and shares call responsibilities. They promote quality care, adhere to organizational policies, maintain licensure and certifications, and collaborate with healthcare teams to ensure optimal outcomes.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, relevant licensing, certifications, outpatient experience, and proficiency with electronic health records. Preferred candidates have 2+ years of family medicine experience and familiarity with Medical Home care models. Compensation ranges from $154,568 to $165,351 annually, with benefits including health insurance, retirement plans, and paid time off. Relocation to Prineville, OR is required prior to start.
