Job Description
The Advanced Practice Provider (APP) provides comprehensive outpatient primary care across all ages under supervision in a Medical Home model.
Responsibilities include evaluation, diagnosis, treatment, documentation, and coordination of medical, behavioral health, pharmacy, and ancillary services. Works at the Prineville Clinic, with a minimum of 40 hours/week and patient contact of at least 36 hours. Participates in shared call rotation, including evenings, weekends, and holidays. Collaborates with a multidisciplinary team to deliver evidence-based, personalized care, and develops treatment plans incorporating various specialties. Maintains documentation, reports diagnostics, educates patients, and supports quality improvement initiatives. Requires a Master’s in Physician Assistant Studies or Nursing, valid licensure, certification (e.g., NCCPA, ANNP), DEA registration, and relevant clinical experience in outpatient family medicine. Preferred experience includes 2+ years in Family Medicine, familiarity with Medical Home models, and Epic EHR.
