Seeking an Advanced Practice Provider to deliver comprehensive outpatient care in a Medical Home setting.
Provide evaluation, diagnosis, treatment, and documentation for patients of all ages. Coordinate medical, behavioral health, nursing, pharmacy, nutrition, and surgical services to optimize outcomes. Create and monitor personalized treatment plans, referring to specialists as needed. Work a minimum of 40 hours/week, including patient contact and participation in shared call rotations, evenings, weekends, and holidays. Practice evidence-based family medicine, collaborate with multidisciplinary teams, and ensure quality and compliance standards. Maintain current licensure, certifications, and documentation, with a focus on patient-centered, ethical care. Qualifications include a Master’s in Nursing or Physician Assistant Studies, relevant licensure, and experience in outpatient family medicine. Preferred candidates have 2+ years of experience, familiarity with Medical Home models, and Epic EHR proficiency. The role offers full-time employment with competitive compensation and benefits.
