Job Description

Seeking an *Advanced Practice Provider* to deliver outpatient family medicine care under supervision.

  • Provide evaluation, diagnosis, treatment, and documentation for patients of all ages within a Medical Home model.
  • Coordinate services with behavioral health, pharmacy, nursing, and other teams to ensure comprehensive, cost-effective care.
  • Create and monitor personalized treatment plans, referring patients to specialists as needed.
  • Work 40+ hours/week with scheduled patient contact and participate in rotating call duties including evenings, weekends, and holidays.
  • Collaborate with multidisciplinary teams to promote evidence-based, patient-centered care and ensure quality outcomes.
  • Maintain current licensure, certifications, and compliance with policies.
