Job Description
Seeking an Advanced Practice Provider to deliver comprehensive outpatient care in a Medical Home setting, supervised by a physician.
Responsibilities include evaluation, diagnosis, treatment, and documentation for patients of all ages, coordinating services across disciplines, creating integrated treatment plans, and monitoring patient progress.
Work schedule: minimum 40 hours/week with at least 36 hours of patient contact; participation in shared call rotation, including evenings, weekends, and holidays.
Department emphasizes team-based, evidence-based, personalized care supported by behavioral health, pharmacy, and care management staff.
Essential duties involve practicing within scope, communicating effectively, prescribing medications, documenting thoroughly, and participating in quality improvement.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, certification (NCCPA or AANC/ANNP), and experience in outpatient Family Medicine. Preferred qualifications include a Doctorate, Oregon licensure, and experience with EHR systems like Epic.
Compensation ranges from $143,784 to $165,352 annually, with benefits such as health insurance, retirement plans, and paid time off.
