Job Description
The Advanced Practice Provider provides comprehensive outpatient care under supervision, including evaluation, diagnosis, treatment, and coordination within a Medical Home model.
Work schedule: minimum 40 hours/week with at least 36 hours patient contact, participating in a shared call rotation including evenings, weekends, and holidays.
Department: Part of a multidisciplinary team delivering evidence-based, personalized care supported by Behavioral Health, pharmacy, care managers, and staff.
Key duties include practicing family medicine, prescribing medications, documenting care, interpreting diagnostics, and patient education. The role emphasizes collaboration, professionalism, quality improvement, and adherence to policies.
Qualifications: Master’s degree in Physician Assistant Studies or Nursing, current licensure, certifications (e.g., DEA, BLS, ACLS if applicable), and outpatient care experience. Preferred credentials include a Doctorate and Oregon licensure.
Compensation: $143,784 – $165,352/year plus benefits such as health insurance, retirement plans, paid time off, and more.
