Job Description
The Advanced Practice Provider (APP) delivers comprehensive outpatient care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation within a Medical Home model.
Works at least 36 hours weekly with scheduled days varying by clinic hours. Participates in a shared call rotation, including evenings, weekends, and holidays, with backup support from a physician. Collaborates with a multidisciplinary team to provide evidence-based, personalized care, incorporating behavioral health, pharmacy, and other services. Prescribes medications, interprets diagnostics, and educates patients on preventive and treatment plans. Maintains documentation, billing, and compliance standards, supporting quality improvement and organizational goals. Requires a Master’s degree in Physician Assistant Studies or Nursing, valid licensure, relevant certifications, and outpatient care experience. Preferred qualifications include a Doctorate, 2+ years in Family Medicine, and Epic EHR experience. Compensation ranges from $143,784 to $165,352 annually, with benefits such as health, dental, vision, retirement plans, and paid time off.
