Job Description

The Advanced Practice Provider (APP) delivers comprehensive outpatient care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation within a Medical Home model.

  • Works at least 36 hours weekly with scheduled days varying by clinic hours.
  • Participates in a shared call rotation, including evenings, weekends, and holidays, with backup support from a physician.
  • Collaborates with a multidisciplinary team to provide evidence-based, personalized care, incorporating behavioral health, pharmacy, and other services.
  • Prescribes medications, interprets diagnostics, and educates patients on preventive and treatment plans.
  • Maintains documentation, billing, and compliance standards, supporting quality improvement and organizational goals.
  • Requires a Master’s degree in Physician Assistant Studies or Nursing, valid licensure, relevant certifications, and outpatient care experience. Preferred qualifications include a Doctorate, 2+ years in Family Medicine, and Epic EHR experience.

  • Compensation ranges from $143,784 to $165,352 annually, with benefits such as health, dental, vision, retirement plans, and paid time off.
