Help underserved community members access quality healthcare through home visits, focusing on high-risk AHCCCS and Medicaid patients with complex needs.
Responsibilities include examining patients, reviewing tests, creating treatment plans, prescribing and reconciling medications, educating families, coordinating care with providers, and managing patient engagement for one year to improve outcomes.
You will work closely with a multidisciplinary team to address care gaps, facilitate transitions, and support patients’ health goals, primarily managing five home visits daily and overseeing a population of 100 members.
Qualifications include licensed Nurse Practitioner credentials, strong communication skills, experience in primary and acute care, and the ability to connect with diverse patients. Preference for Spanish speakers.
Position covers the Yavapai East area, with a starting salary of $130,000 and benefits such as insurance, 401(k), paid time off, and performance bonuses up to 20%. Flexible, full-time role with on-the-road work environment.
