Job Description

Help vulnerable communities access quality healthcare by providing in-home care to high-risk Medicaid and AHCCCS members.

  • Conduct patient examinations, review medical records, and interpret diagnostic tests.
  • Develop and implement integrated treatment plans, prescribe and reconcile medications.
  • Educate patients and families, coordinate care with providers, and oversee transitional procedures.
  • Manage patient engagement, close care gaps, and collaborate with team members including social workers and specialists.
  • Work with a team to monitor progress, support patients’ goals, and improve health outcomes over a year-long engagement.

  • Requirements include licensed Nurse Practitioner certification, strong communication skills, and ability to connect with patients. The role covers the Yavapai East area, with a typical schedule of managing 5 patients daily. Compensation starts at $130,000 annually, with benefits including insurance, 401(k), and bonuses.
