Help underserved populations engage with healthcare by providing in-home primary and chronic care as a Nurse Practitioner. You will manage high-risk Medicaid and AHCCCS members, coordinating care, educating families, and collaborating with healthcare providers to improve medical outcomes over a one-year period.

  • Conduct patient exams, review records, create treatment plans, prescribe and reconcile medications.
  • Educate members and families, oversee care engagement, and ensure smooth transitions from facilities.
  • Work closely with multidisciplinary teams including medical assistants, social workers, and specialists.
  • Manage approximately 100 members, visiting five patients daily in their homes, focusing on goal achievement and barrier reduction.
  • Maintain accurate documentation and follow healthcare regulations.
  • Qualifications include licensed Nurse Practitioner certification, strong communication skills, and experience in primary or acute care. Bilingual in Spanish is preferred. Compensation starts at $130,000 annually with benefits such as insurance, retirement plans, and performance bonuses.
