Job Description
Nurse Practitioners in this role provide in-home care to high-risk, high-cost Medicaid and AHCCCS patients, focusing on coordination, education, and goal-setting to improve health outcomes.
Responsibilities include examining patients, reviewing diagnostics, creating treatment plans, prescribing and reconciling medications, maintaining records, and collaborating with other healthcare providers. They engage patients and families, oversee care gaps, and work as part of a multidisciplinary team to ensure comprehensive care.
The position involves managing approximately 100 members monthly, visiting five patients daily in their homes across the Yavapai East area, and supporting transitional care and ongoing engagement.
Applicants must hold a valid Nurse Practitioner license in Arizona and demonstrate experience in primary, acute, or chronic care. The role offers a starting salary of $130,000, with benefits including insurance, 401(k), bonuses, and paid time off. Bilingual skills in Spanish are preferred.
