Help the most vulnerable community members access quality healthcare by providing in-home care, education, and care coordination.
Manage high-risk patients’ medical needs, create treatment plans, prescribe and reconcile medications, and document encounters. Educate patients and families, oversee engagement with healthcare providers, and facilitate transitional care. Collaborate with a multidisciplinary team including medical assistants, social workers, and specialists to improve health outcomes. Visit approximately five patients daily, managing a caseload of 100 members over a year, with a focus on goal achievement and barrier reduction. Require a valid Nurse Practitioner license, preferably bilingual (Spanish), with experience in primary, acute, or chronic care. Offer a starting salary of $130,000+, with benefits including insurance, retirement, paid time off, and performance bonuses. This full-time role covers the Yavapai East area, emphasizing compassionate, team-oriented care for high-need populations.
