Job Description
Assist high-risk, high-cost Medicaid and AHCCCS patients in their homes to improve health outcomes and reduce barriers to care.
Work closely with patients, families, and healthcare providers to coordinate care, educate on medical needs, and develop personalized treatment plans.
Manage approximately 5 patients daily, conducting assessments, reviewing tests, prescribing medications, and ensuring follow-up.
Collaborate with team members including medical assistants, social workers, and behavioral health specialists to support patient goals and transitional care.
Maintain accurate records and engage patients in goal-setting to promote better health and life quality.
Position covers specific geographic areas, with compensation starting at $130,000 annually plus benefits and performance bonuses.
Requires a valid Nurse Practitioner license, excellent communication skills, and experience in primary or chronic care.
Spanish proficiency preferred.
