Help high-risk, high-cost Medicaid and AHCCCS patients in their homes to improve health outcomes through care coordination, education, and goal setting.
Requires proven NP experience, strong communication skills, patient engagement ability, and valid licensure in Arizona. The role covers the Yavapai East area, with a typical schedule of managing about 100 members and visiting five patients daily in their homes. Compensation starts at $130,000/year, with benefits including insurance, retirement plans, paid time off, and performance bonuses. Bilingual in Spanish is preferred.
