Job Description

Assist high-need, high-risk Medicaid and AHCCCS members in their homes to improve health outcomes and meet personal goals.

  • Conduct patient exams, review records, and interpret diagnostic tests.
  • Create and implement treatment plans, prescribe and reconcile medications.
  • Educate patients and families on medical needs and facilitate engagement with healthcare providers.
  • Coordinate care with primary care, specialists, case managers, and behavioral health teams.
  • Administer transitional care and address care gaps to ensure seamless service.
  • Work with a team to plan patient engagement, monitor progress, and support life improvements.

  • Full-time role covering the Yavapai East area, managing about 100 members monthly, seeing five patients daily in their homes. Compensation starts at $130,000/year plus benefits, bonuses, and insurance. Requires an Arizona licensed Nurse Practitioner, excellent communication skills, and ability to connect with diverse patients. Bilingual (Spanish) preferred. The position offers a collaborative environment focused on community health and patient-centered care.
