Help the most vulnerable community members access quality healthcare through in-home visits, care coordination, and patient education.

Responsibilities include examining patients, reviewing tests, creating treatment plans, prescribing and reconciling medications, maintaining records, and collaborating with healthcare teams to improve outcomes.

Work involves managing a population of 100 members, visiting five patients daily, and partnering with team members like medical assistants, social workers, and behavioral health specialists.

Requirements include valid Nurse Practitioner licensure, experience in primary, acute, or chronic care, strong communication skills, and a patient-centered approach.
