Job Description
Help vulnerable communities access quality healthcare by providing in-home care to high-risk Medicaid and AHCCCS members.
As a Nurse Practitioner, you will assess patients, review tests, create treatment plans, prescribe medications, educate families, and coordinate care with providers and team members. You will manage a caseload of 100 members, visiting five patients daily, and work closely with medical assistants, social workers, and behavioral health specialists to improve health outcomes and address barriers.
Responsibilities include patient examinations, medication reconciliation, care planning, transitional care, and engaging patients around their health goals.
Qualifications involve nursing licensure, experience in primary or acute care, excellent communication, and teamwork skills.
Position covers the Yavapai East area, with a starting salary of $130,000 plus benefits such as insurance, 401(k), paid time off, and bonuses.
