Join our team as a Nurse Practitioner dedicated to serving high-risk, high-need patients in their homes to improve health outcomes and quality of life.
Responsibilities include:
- Examining patients and reviewing medical records
- Creating and managing treatment plans with integrated medical practices
- Prescribing and reconciling medications
- Educating patients and families about their healthcare needs
- Coordinating care with providers and overseeing patient engagement
- Managing gaps in care and ensuring smooth transitions from facilities
- Collaborating with team members such as medical assistants, social workers, and specialists
Requirements:
- Valid Nurse Practitioner license (Arizona-specific)
- Experience in primary, acute, or chronic care
- Strong communication, ethical standards, and patient engagement skills
- Ability to work collaboratively in a team environment
Work Details:
- Manage approximately 5 home visits daily, with a panel of 100 members
- Coordinate care planning and follow-up over a year
- Position covers the Yavapai East area (e.g., Sedona, Cottonwood, Prescott Valley)
- Competitive starting salary from $130,000/year plus benefits and bonuses
