Key Responsibilities
Conduct assessments and develop personalized wound care plans
Deliver bedside wound care and lead advanced procedures
Utilize technology and protocols to promote healing
Collaborate with facility teams and families for holistic care decisions
Maintain accurate EMR documentation and support quality improvements
Qualifications
Active PA Nurse Practitioner license or Physician Assistant certification
Prescriptive and DEA licenses required by end of training
Minimum 2 years wound care experience, preferably in SNFs or long-term care
Willingness to travel within designated counties
Strong communication and collaborative skills
