Home-Based Therapist

Job Description

We are seeking a compassionate Home-Based Therapist to provide in-home therapy for children, adolescents, and families in Bad Axe, Michigan.

Offer clinical assessments, develop and monitor individualized family plans, and conduct regular home visits to support parenting and family relationships.

Maintain face-to-face contact, act as a liaison with community agencies, and provide crisis stabilization and resource linkage.

Participate in team planning, supervision, and community education.

Qualifications: Master’s degree in Social Work, Counseling, or Psychology; Michigan licensure (LMSW or LLMSW); minimum 2 years’ experience preferred; valid driver’s license; willingness to obtain Infant Mental Health endorsement.
