Job Description
We are seeking a Speech Language Pathologist to join our Home Care team, providing personalized evaluation and treatment in patients’ homes to improve communication, cognition, and swallowing abilities.
Conduct assessments, develop evidence-based treatment plans, and adjust strategies based on patient progress.
Educate patients and caregivers on communication techniques and safe swallowing practices.
Collaborate with an interdisciplinary team, participate in care planning, and document all interventions to ensure compliance.
Maintain licensure, stay current with clinical best practices, and contribute to team growth.
Qualifications include a Master’s degree, current NY State licensure, CCC from ASHA or CFY, and CPR certification. Prior experience with dysphagia and neurogenic disorders is preferred.
