Job Description

Seeking a Speech-Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, enhancing communication, cognition, and swallowing abilities.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based treatment plans, and adjusting strategies based on patient progress.

Ensure thorough documentation, educate patients and caregivers, collaborate with interdisciplinary teams, and support care planning and equipment coordination.

Qualifications include a Master’s in Speech-Language Pathology, CCC or CFY, NYS licensure, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
