Job Description

Seeking a Speech Language Pathologist for home care, providing personalized evaluations and treatments to improve communication, cognition, and swallowing in patients’ homes.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based care plans, and adjusting strategies based on patient progress. Documentation, patient and caregiver education, collaboration with team members, and participation in care planning are essential.

Qualifications include a Master’s in Speech-Language Pathology, CCC or CFY status, NY State licensure, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
