Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, enhancing communication, cognition, and swallowing abilities.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers. Collaboration with a multidisciplinary team and participation in care planning are essential.

Qualifications:

• Master’s in Speech-Language Pathology
• CCC from ASHA or CFY candidate
• NY State licensure
• CPR certification
