Job Description

Seeking a Speech-Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.

This role involves conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers. You will collaborate with an interdisciplinary team, participate in care planning, and ensure compliance with regulations.

Requirements include a Master’s in Speech-Language Pathology, CCC or CFY, NY licensure, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
