Job Description
Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Conduct assessments, develop and adjust evidence-based treatment plans, and document patient progress in compliance with regulations.
Educate patients and caregivers on communication and swallowing strategies, fostering strong rapport and collaboration with interdisciplinary teams.
Collaborate with care providers to coordinate equipment and resources, maintain licensure, and stay current with clinical practices.
Qualifications include a Master’s degree, CCC or CFY, NY State licensure, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
