Join our healthcare team as a part-time Home Health Nurse Practitioner, providing compassionate care directly in patients’ homes.

Responsibilities include conducting assessments, developing care plans, administering treatments, performing clinical procedures, and educating patients on disease management and lifestyle modifications. You will utilize EHR systems like Epic or eClinicalWorks and coordinate with multidisciplinary teams to ensure seamless care.

Requirements include a current Nurse Practitioner license, experience in critical care preferred, proficiency in procedures such as spinal taps and airway management, and strong assessment and communication skills. Experience with telehealth and diverse populations is a plus.
